Please don’t let this client form intimidate you. Providers that say on their ad “text me for more info” “text me for rates” screen you thru text instead and ask you similar Q&A’s that I’m asking of you on my client form.
Each their own but this is how I decide to screen new clients.

My preferences on Availability:

I do not have a guest over and I do not provide outcalls past 7 PM start time and I do not make exceptions.

Deposit Info:

I do ask all new clients to send a small deposit to secure your time with me and to also show your dependability.

I use Cash app or PayPal.
If Sending a deposit is a NO for you please do not submit my client form. 
 

It’s unfortunate that I have to spell out basic logical stipulations in black-and-white but I do.
If you submit this client contact form and you are trying to move forward in scheduling a date, then I send you the deposit information, and it takes you an unreasonable timeframe to send it, aka you do not send the deposit at all, I no longer see you as dependable and your appointment is canceled. 
 

Once you receive my deposit information, I expect to receive the small baby deposit within the same business day. I only ask once for the deposit.
If you attempt to purposefully lag/swindle me/try and play games with me you’re blocked and you're appointment is politely canceled.

I accept Cashapp and PayPal. If digital app's do not work for you I do have alternative deposit options.
